I understand that I will be billed bottles of wine every other month unless I request a hold. I certify that I am 21 years of age or older. I give permission to Lost Oak Winery to charge my credit card every other month, and understand that my card will be billed for shipping and/or handling. If my package is not picked up in a timely manner, I authorize shipment of my wines at current shipping rates plus a repacking fee. I am aware that I may cancel at anytime by phone, e-mail or in writing two weeks prior to a scheduled shipment. In addition, I agree to provide current contact and billing information, if no contact is made in a four month period, Lost Oak Winery reserves the right to terminate this membership.
